Acknowledgment and Assumption of Risk

I understand that participation in youth basketball, cheerleading, practices, games, clinics, team activities, and related events involves inherent risks. These risks may include falls; collisions with people, equipment, or playing surfaces; strenuous physical activity; equipment failure; facility conditions; and other known or unexpected hazards. Injuries may include sprains, fractures, concussions, serious bodily injury, permanent disability, or death.

I knowingly and voluntarily accept these risks on behalf of the participant and confirm that the participant is physically able to participate, except for any limitations disclosed to the organization in writing.

Release and Hold Harmless

To the fullest extent permitted by North Carolina law, I release and agree to hold harmless the Tabernacle Community Association, the Tabernacle Cougars program, and their officers, directors, administrators, coaches, volunteers, agents, representatives, participating schools, facility owners, and event hosts from claims arising from the participant’s involvement in program activities, including claims based on ordinary negligence.

This release does not apply to conduct that cannot lawfully be waived, including gross negligence or intentional misconduct. I understand that this agreement does not guarantee that an injury will not occur.

Emergency Medical Authorization

If I cannot be reached during an emergency, I authorize program representatives to obtain reasonable emergency medical care for the participant. I understand that I am responsible for medical costs not covered by insurance and that the organization does not provide individual health or accident insurance unless stated separately in writing.

Rules, Safety, and Conduct

I agree that the participant will follow program rules, coach instructions, safety requirements, and the Tabernacle Cougars code of conduct. I understand that the participant may be removed from an activity when program representatives reasonably believe continued participation presents a safety or conduct concern.

Entire Agreement and Severability

This waiver applies to all Tabernacle Cougars basketball and cheer activities for the 2026–27 season. If any part is found unenforceable, the remaining provisions will continue to the extent allowed by law. North Carolina law governs this agreement.
